Interested and reputable Suppliers, Contractors, and Consultants must submit their firm’s profiles containing the following documents:
Evidence of Certificate of Incorporation (CAC) – Including Form CAC1, CAC2, and CAC7 (BN1 acceptable for Services).
Tax Clearance Certificate – Company’s Income Tax Clearance (or Personal Tax Clearance for Partners) for the last three (3) years (2022, 2023, 2024) valid till 31st December 2025.
Pension Clearance Certificate – Valid till 31st December 2025 (Required for firms with 15+ staff).
Industrial Training Fund (ITF) Compliance Certificate – Valid till 31st December 2025 (Required for firms with 5+ staff or N500m+ annual turnover for LOT 1 & LOT 5).
NSITF Clearance Certificate – Valid till 31st December 2025.
BPP Interim Registration Report (IRR) or Valid BPP Certificate expiring on 31/12/2025.
Sworn Affidavit disclosing:
No conflict of interest with FUHSA/BPP officials.
No criminal conviction of directors for fraud/financial crimes.
Company is not under insolvency/winding-up proceedings.
Audited Accounts (2022, 2023, 2024).
Bank Reference Letter (confirming credit facility availability).
COREN Certification (for Construction Lots) + Staff CVs (COREN/QSRBN/ARCON/CORBON certified).
Three (3) Similar Projects Evidence (Letters of Award, Completion Certificates, Photos).
For LOT 12 (Vehicles): Authorization letter from original manufacturers.
For Medical Services (LOT 17): Current MDCN/Relevant Council Practice License.
For Joint Ventures (JV): MoU + Separate CAC/Tax/Pension/ITF/NSITF/IRR for each partner.
Non-refundable Fee: ₦15,000 per Lot (Paid into FUHSA’s TSA Account).

Expression of Interest (EOI) Deadline: 12:00 noon, Monday, 9th June 2025.
Technical Bid Deadline: 12:00 noon, Monday, 23rd June 2025.
